Cardiovascular disease (CVD) has an enormous impact on the lives and health of New Zealanders.

In 2008, Dr Sue Wells and Dr Andrew Kerr came up with a novel approach to support CVD risk communication and behaviour change; 'Your Heart Forecast'.

Estimating the risk of CVD events such as a heart attack or stroke in the next five years is relatively easy using fully-fledged clinical cardiovascular risk assessment tools (such as Predict, best practice, The Edge, Mohio etc.).  However, practitioners can find it difficult to explain to patients the concept of CVD risk, and the effect lifestyle and behaviour changes have on risk factors. 

Your Heart Forecast bridges this gap and aids health professionals to convey cardiovascular risk using a graphically based story that shows patients:

  • An estimate of their current risk (where they are now) – or their actual risk if the results are pre-populated across from one of the risk assessment tools outlined above
  • How it relates to a peer with ideal risk factor control and same CVD risk (their cardiovascular age)
  • What would happen to their risk as they get older and made no changes (their heart forecast), and conversely
  • What would happen to their risk if they made healthy lifestyle changes; for example, stopped smoking

Consumer vs Clinician Versions

There are two versions of this tool for use in two key settings, both provide a graphical story but are designed for different uses.

Clinicians version - (refer below for access links)
Provides a risk estimate and is for use in consultation with patients.

Consumer version - ' Know Your Numbers' website
Provides a risk estimate and is intended for use by the general public without the support of a health professional.

There are three ways for health professionals to access this tool.

Clinical/Health Professional Version:  This is the preferred method.  Access it directly through a fully-fledged risk assessment tool (such as Predict, best practice, The Edge, Mohio etc.)

Use this tool online now: If using the tool regularly, you will be asked to register to use this version via a simple registration process. Registration will be granted if your use is in accordance with the conditions below. Why do I need to register?

Download this tool now: Only available to those who cannot use the online version of the tool. This version is regularly updated and your download will expire at a set date to ensure that the most recent version is utilised.


Free Use Criteria

To use this tool free of charge you must meet the following criteria:

  • You are based in, and intend to use the tool in New Zealand
  • You work within the healthcare sector of New Zealand (including clinical pharmacists with access to CVDRA results from a accredited risk assessment tool)
  • You use this tool alongside an established and fully qualified NZ Risk Assessment tool - Why?
  • You are not going to charge for the use of this tool
  • You are not servicing a corporate or workplace
  • You are not offering this tool within a bundle of services which are being charged for
  • You understand the ethical obligations of risk screening and will provide the necessary follow-up and referral into General Practice for anyone found to be at a manageable level of risk
