Your Heart Forecast

The Your Heart Forecast tool was designed by Drs Sue Wells and Andrew Kerr, at the University of Auckland, and supported by the Heart Foundation, to help doctors and nurses address that challenge using a graphic story.

The graphic story shows patients:

• Their current risk (where they are now)
• How it relates to a peer with ideal risk factor control and same CVD risk (their cardiovascular age)
• What would happen to their risk as they get older and made no changes (their heart forecast), and conversely
• What would happen to their risk if they made healthy lifestyle changes, for example, stopped smoking.

This tool is available without charge to a restricted set of users in NZ, in order to use this tool from our site you must meet the following criteria:

• You are based in, and intend to use the tool in New Zealand.
• You work within the HealthCare sector of New Zealand (including Clinical Pharmacists)
• You are not going to charge for the use of this tool.
• You are not servicing a corporate or workplace.
• You are not offering Your Heart Forecast within a bundle of services which are being charged for.
• You understand the ethical obligations of risk screening and will provide the necessary follow-up and referral into General Practice for anyone found to be at a manageable level of risk.
