This tool is available without charge to a restricted set of users in NZ, in order to use this tool from our site you must meet the following criteria:

• You are based in, and intend to use the tool in New Zealand.
• You work within the HealthCare sector of New Zealand (including Pharmacy workers)
• You are not going to charge for the use of this tool.
• You are not servicing a corporate or workplace.
• You are not offering Your Heart Forecast within a bundle of services which are being charged for.
• You understand the ethical obligations of risk screening and will provide the necessary follow-up and referral into General Practice for anyone found to be at a managable level of risk.
